Making a referral to Holy Cross Hospital

Once you have confirmed that the relevant Clinical Commissioning Group (CCG) is supportive of the referral, please contact our Director of Patient Services and complete a referral form. For outpatient physiotherapy or hydrotherapy, please visit our Physiotherapy Centre‘s website. 

Once we have received the referral for Holy Cross, we will:

• Undertake a detailed pre-admission assessment 
• Encourage prospective patients and/or their families to visit 
• Prepare a comprehensive report and preliminary risk assessment including details on proposed fees

A funding agreement must be in place before admission.
