Refer your patient by completing our referral form.

Before starting to complete the form, we recommend you export it and save it on your system.

What happens after we receive your referral

We will:

• undertake a detailed pre-admission assessment
• encourage prospective patients and/or their families to visit
• prepare a comprehensive report and preliminary risk assessment including details on proposed fees.
• confirm with you that a funding agreement is in place before admission.

Outpatients

Refer your patient for outpatient physiotherapy or hydrotherapy by contacting the physiotherapy clinic.
